CAMP SHUTTLE INFORMATION FORM

PARENTS - if your child is using the camp shuttle service, please fill out the appropriate section below, and send this back to us at least three (3) weeks before the start of the camp session. Enclose the $40.00 round trip fee or $20.00 one-way fee. Thank you.

IMPORTANT: A camp counselor wearing a COLORADO SOCCER CAMP T-shirt will meet your child at the baggage claim. Please tell your daughter or son to wait for the counselor, who will then take him or her to the shuttle van. Please be sure that your child has the camp telephone number, (303) 735-0350, in case he or she arrives early or late.

ARRIVAL: FIRST DAY OF CAMP, ANYTIME BETWEEN 6:00AM AND NOON
DEPARTURE: LAST DAY OF CAMP, BETWEEN 1:00PM AND 4:00PM
